The following has fixed it for me every time so far.
1. Download and install Asio4All.
2. Launch Pro Tools. Press and hold ‘N’ during the Pro Tools launch process.
3. This will launch Pro Tools into the Playback Engine window.
4. Select Asio4All as the Playback Device. Press ‘Ok’.
5. At this point you can now go back into the Playback Engine window (Setup > Playback Engine) and select the desired interface.
