There are at least two versions of the OS 9.1 install disks, version 1.2 and 1.3.  Version 1.3 installs Mac OS ROM version 7.9.1.  7.9.1 causes the Type 10 errors in Pro Tools 5.1U.  Initializing the drive and installing OS 9.1 from the 1.2 installer disk resolves this issue.
