Using a PC to control midi on synths

Hope someone out there can help.
So far my use of midi has been to use a trigger keyboard to operate a synth module - that's all fine and works!
But for the next project I'm working on I need to be able to move things on a bit!
Scenario:
I have a standalone synth and a synth module controlled by a mid trigger keyboard. I have some very fast registration changes and can't be fiddling around entering patch numbers or twiddling knobs!
Is it possible to use some midi controller software on a PC (I have an Edirol USB>Midi adaptor) to set up the patch changes in sequence so that I can trigger a patch change with a mouse click or spacebar hit?
Any ideas please?
